Ejection / Incident Report
Submit the Ejection Report by 10:00 PM on the day of the ejection and call the assigner immediately. Complete the form online, provide necessary details, and manage post-ejection records as required.

Steps to Complete the Ejection Report:
Visit the designated website by clicking the button below.
Select your sport and enter the password: “123”.
Click on Ejection Misconduct to access the form.
Complete all required fields in the form.
The completed form will be sent to both the CIFLA office and the offending school.
Indicate if a post-ejection report or letter is necessary to issue a suspension.
If needed, the school can upload the post-ejection report or letter, after which you can finalize the misconduct record.
